Osteoarthritis is a chronic, degenerative arthropathy (affecting approximately 70% of the population over 65) resulting from an imbalance between chondroformation (cartilage formation) and chondrolysis (cartilage degeneration). Treatment requires two substances that modulate cartilage structure: glucosamine and chondroitin. Articular cartilage is composed of water (65 to 75%), collagen fibers (which give it elasticity and shock absorption), proteoglycans, mucosaccharides, large hydrophilic protein molecules, and carbohydrates in the form of chains housed within the collagen fibers, formed by the polarization of chondroitin-sulfuric acid combined with glucoronic acid and glucosamine. They are essential for collagen health by capturing and retaining water for cartilage lubrication, which allows for its strength. The cartilage matrix contains chondrocytes (10% total), stem cells (undifferentiated) that produce new collagen cells, and proteoglycans that release enzymes that digest aging cells. Chondroformation and chondroabsorption are balanced. With age, an imbalance occurs that leads to osteoarthritis, due to the lack of collagen maintenance and cartilage fragility, which causes fissures, the appearance of pseudocysts or geodes, and synovitis.

Glucosamine Sulfate (glucose associated with an amino acid, glutamine) participates in the production of proteoglycans with a direct effect on the synthesis of mucosaccharides by chondrocytes.
Inhibits the activity of collagenase (an enzyme that degrades collagen). It is an important element in the composition of mucosaccharides, a fundamental substance in the structure of cartilage. It is present in bones, skin, and tissues. It has anti-inflammatory properties, reducing the functional index, pain, and edema.

Chondroitin sulfate (a mucopolysaccharide composed of N-acetylgalactosamine associated with an amino sugar, glucuronic acid) captures and transports water to proteoglycans. It inhibits hyaluronidase, an enzyme that reduces the viscosity of hyaluronic acids.
Chondroprotective agent, as it maintains osmotic pressure and preserves cartilage:
Cartilage-damaging enzymes, such as collagenase and elastase, reduce the activity of glucosamidase, an enzyme that breaks down glucosamine. This substance is also found in bones, skin, cornea, and arterial membranes.
From free radical damage, including nitric oxide emitted by chondrocytes. The synergy of these two substances enhances the activity of hyaluronic acid present in synovial fluid.

Phenolic glycosides, especially salicylic acid from Meadowsweet (Filipendula ulmaria), have anti-inflammatory and analgesic properties due to their uric acid-eliminating properties. Salicylic acid has a recognized antispasmodic effect.
